 I understand that the purpose of these educational classes is to learn about Natural Eyesight and Health Improvement.  It is an opportunity to learn the habits of normal eyesight and healthy living.  I understand that I am responsible for integrating what is taught into my life in order to benefit from this educational program.    I understand that this course is not diagnosis, treatment, prescribing, therapy, counseling, or determination of function.  I understand that Natural Eyesight Improvement and Herbal Teachers are not opticians, optometrists, ophthalmologists, physicians, psychologists, or therapists; they are teachers.  I am advised to have an eye doctor monitor my progress and prescribe weakened lenses when needed and a physician to monitor any health concerns I have.  I agree to inform Natural Health Renewal LLC about any and all eye surgeries, including refractive corneal, or lasik performed on me before enrollment.  I agree to disclose any health problems  I have had such as diabetes, heart attack, stroke, high blood pressure, cancer, AIDS, ect...

   I understand that I am solely responsible for my own responses, reactions, and feelings to the class material presented in the Natural Health Renewal retreat.  I understand that self-healing with a natural holistic approach can take from several months to several years, or more.  I agree to hold harmless and indemnify the teachers, staff, and Natural Health Renewal LLC from any and all liability for personal injury, death, or property damage arising out of activities thereunder or in result of consequences thereof.   I am here at this retreat on my own behalf, and not as an agent for any agency.   I agree to give Natural Health Renewal LLC permission to use records of improvement, statements, recordings and pictures obtained during the retreat for testimonials, literature, and advertising; unless waived in writing at time of registration.
